docs(changelog): rewrite the last thirty releases for readers

Releases 1.13.1 through 1.22.1 are regrouped by what each change is
(New, Improvements, Fixes, Misc) and reworded in the voice the
changelog-authoring skill asks for: one or two plain sentences per
bullet, the symptom or the capability named, no mechanism. Packed
bullets were split, every fact and every contributor credit kept.
Older releases keep the mechanical grouping.

## App
### New

### Improvements
- Chat: selecting a user-installed skill from the slash command menu now invokes the skill instead of inserting the skill name as plain text (thanks to @IbrahimKhan12).
- Chat: code blocks in user messages now preserve characters like `<` and `->` instead of escaping them inside the code block (thanks to @bashrusakh).

- Settings/Skills: the skills catalog refreshes after catalog settings change (thanks to @gokulkgm).

- Git: pushing from the Git view now syncs first, reducing rejected pushes when the branch needs to update.
- Usage: MiniMax M3 and Token Plan usage now handle the provider's latest API response format (thanks to @baruchvitorino).
- VSCode: font size and padding preferences now apply inside the extension webview (thanks to @Sin991114).
- Files: downloads and file names with non-Latin characters now handle those characters correctly in headers (thanks to @FanFan4204).
- Agents: clearing temperature or top-p removes the override (thanks to @bashrusakh).
- Git: pushing from the Git view syncs first, so fewer pushes come back rejected.
- Settings: your font size and padding apply inside the VS Code extension too (thanks to @Sin991114).
### Fixes
- Chat: pasted text containing `@` no longer opens file mention autocomplete unexpectedly (thanks to @charpeni).
- Chat: switching sessions and loading older messages no longer causes the conversation to jump backward or oscillate around the current scroll position (thanks to @herjarsa).
- Sessions: new sessions now stay attached to the selected project or current workspace directory instead of sometimes appearing under a stale project (thanks to @bashrusakh).
- Sessions: pinned sessions and folder rows no longer disappear from the sidebar after an empty session-list refresh (thanks to @bashrusakh).
- Startup: managed OpenCode server processes left behind by a previous crash are cleaned up on the next start.
- CLI: stale server PID files are checked more carefully so unrelated processes are not mistaken for an OpenChamber server.
- Mobile: subagent chevrons no longer overlap long session titles, and session grouping now matches the exact workspace directory (thanks to @weixiang1862, @lilyzhaun).
- Chat: picking one of your own skills from the slash menu runs the skill instead of typing its name into the message (thanks to @IbrahimKhan12).
- Chat: a code block in your own message keeps characters like `<` and `->` (thanks to @bashrusakh).
- Chat: Arrow Up opens prompt history again when the cursor sits at the start of the composer.
- Chat: pasting text with an `@` in it no longer pops up file mentions (thanks to @charpeni).
- Chat: switching sessions or loading older messages no longer jumps the conversation backward or makes it wobble (thanks to @herjarsa).
- Sessions: a new session stays with the project or folder you picked (thanks to @bashrusakh).
- Sessions: pinned sessions and folders stay in the sidebar when a session list comes back empty (thanks to @bashrusakh).
- Settings/Models: per-model visibility and sibling model choices stay saved (thanks to @attilaszasz).
- Settings/Skills: the skills catalog reloads after you change catalog settings (thanks to @gokulkgm).
- Providers: disconnecting a provider from settings works.
- Usage: MiniMax M3 and Token Plan usage read the provider's current responses again (thanks to @baruchvitorino).
- Files: downloading a file with non-Latin characters in its name keeps the name (thanks to @FanFan4204).
- Startup: managed OpenCode processes left over from a crash are cleaned up on the next start.
- CLI: a leftover server PID file no longer makes the CLI treat an unrelated process as OpenChamber.
- Mobile: subagent chevrons no longer sit on top of a long session title (thanks to @weixiang1862, @lilyzhaun).
- Mobile: sessions are grouped by their exact folder (thanks to @weixiang1862, @lilyzhaun).
