refactor(quota): secure managed provider credentials (#2160)
- add shared owner-only credential storage for OpenCode Go, Ollama Cloud, and Cursor
- validate credentials before atomic writes using 0700 directories and 0600 files
- replace provider-specific credential routes with an allowlisted lifecycle API
- stop automatically reading Ollama's legacy cookie file
- stop reading or modifying Cursor's database during regular quota requests
- add explicit one-time Cursor credential import without mutating Cursor storage
- persist refreshed Cursor credentials only in OpenChamber-managed storage
- add Ollama Cloud and Cursor credential controls to provider settings
- preserve OpenCode Go tracking through the shared credential flow
- add VS Code credential management and Cursor quota parity
- reject authentication redirects, enforce request timeouts, and fail on unparseable usage pages
- mask stored secrets in API responses and extend quota security coverage
- update quota provider documentation

chore: remove dead code (59 unused files + ~125 unused exports) (#1835)
* chore: remove dead/unreferenced files across ui, vscode
Remove 59 unused source files (components, hooks, lib utils, stores,
barrels, and orphaned vscode github modules) that are not imported by
any entry-reachable code. Also drop a stale test mock for the removed
execCommands module.

Remove exported symbols whose identifier is referenced nowhere in the
repository (verified via repo-wide search), across ui types/contracts,
lib utilities, sync layer, stores, and components. Also drop the few
imports/private helpers orphaned by these removals.

Continue removing repo-wide unreferenced exported functions, consts and

feat: add Ollama Cloud quota provider (#544)
* feat: add Ollama Cloud quota provider
Adds ollama-cloud quota provider that reads auth cookie from
~/.config/ollama-quota/cookie and fetches usage from ollama.com/settings.
Returns 3 windows: session, weekly, and premium (X/Y). Reuses Ollama
logo via provider alias.
* docs(quota): add ollama-cloud provider entry to DOCUMENTATION.md
