* feat(worktrees): fetch remote source branch before worktree creation

New worktrees based on a local branch that is behind its upstream now
fetch first and branch from the remote-tracking ref, so they are not
born stale. A global setting (on by default) in Settings > Behavior
controls this, and fetch failures toast a warning and fall back to
local state instead of blocking creation.

* fix(worktrees): wire fetch-source toggle to store and honor failed runtime fetches

The Behavior toggle only persisted the setting; the consumer reads the
config store at creation time, so a just-toggled-off setting kept
fetching until the next hydration. Update the store optimistically on
toggle and on page load, and roll it back when the save fails.

The VS Code runtime bridge resolves git fetches with { success: false }
instead of throwing, which the consumer read as success and silently
based the worktree on the stale remote ref. Treat any non-success
result as a failed fetch: warn and fall back to local state, matching
the web/desktop/mobile path.

* fix(worktrees): stop new remote-based worktrees from tracking the base branch

Creating a worktree with a remote start ref made git auto-track the
base branch (branch.autoSetupMerge), so with the new remote fetch every
behind-root worktree was born with upstream origin/<base> and plain
git push refused under push.default=simple.

The new branch's own upstream does not exist until its first push, and
the bootstrap deliberately refuses to write tracking config for refs
that were never fetched, so --set-upstream-to cannot re-point it.
Suppress the auto-track with --no-track on new-mode creation from a
remote ref: the branch ships with no upstream, matching the behavior
before the remote fetch until the first push sets it. Explicit
upstream keys now also win over the remote start ref inference,
aligning the create path with the validate path and the VS Code
runtime.

* fix(worktrees): keep the pre-create remote ref refresh soft

The client fetch and the server's pre-create fetchRemoteBranchRef both
refresh the same branch, and the second fetch throws on failure — so a
connection dropped between the two turned the promised soft fallback
into a rejected creation even though the remote-tracking ref was
already available locally.

The refresh is now best-effort when the ref exists locally (creation
proceeds from it) and still mandatory when the ref was never fetched,
preserving the materialization behavior for remote-only branches.
Applied to both the web server and the VS Code runtime.

* chore: ignore the .openchamber app runtime state directory

Bohdan Triapitsyn 65054a5f4a feat(ui): gate the pull-request surface on GitHub, move the account into it, and GitHub sign-in into Integrations
The pull-request rail icon now appears only while GitHub is connected
(OAuth or gh CLI), like Linear; Linear sits after the walkthrough in the
default rail order. The GitHub account avatar and switcher leave the
header for the pull-request panel, where the walkthrough, refresh, and
account controls share one row and one height, and the account stays
visible on the panel's empty state. A manual refresh keeps its spinner on
screen long enough to read as work done.

GitHub sign-in moves from Settings → Git to Settings → Integrations →
Built-in integrations as a card before Linear; search and the connect
buttons follow it.

Bohdan Triapitsyn 391f938334 feat(voice): match local and macOS voices to the language of the text
Text-to-speech picked one voice regardless of what language a reply was in.
A dependency-free language detector (script, marker letters, function words)
now decides the language of the whole message once; with the new
"Match the voice to the language of the text" setting the local provider
switches to a catalog model for that language (Kokoro zh/en and Piper models
for 12 languages, downloaded on first use like the existing model) and macOS
say switches to an installed voice whose locale matches. The local voice
picker lists voices of every installed model, and the settings show which
language models are on disk.

The Ukrainian Piper medium build is a character-level model that sherpa-onnx
turns into noise, so the espeak-based Lada build is used instead.

* feat(linear): start sessions from Linear issues
Authorize a Linear workspace on this OpenChamber server, map teams to
projects, attach an issue from chat, start a session or worktree from an
issue, and post started/completed/failed comments that open the session.
Hidden in VS Code.

* feat(linear): connect more than one Linear workspace

Store each OAuth grant on this OpenChamber server and keep one current, so Settings can add and switch workspaces without dropping the others. Project mapping is per workspace. Remove the Linear button next to New Chat; start-from-issue stays on New Worktree.

* feat(linear): add a right-hand issues panel

Browse and filter issues in the rail, open a card to change status or start a session, and collapse search plus most filters to icons on a narrow panel.

* feat(linear): open issues in the rail and filter by Linear status

The rail icon only shows after Linear is connected. Clicking a Linear row on work status opens the panel. Status options match the card, including Done, Canceled, and Duplicate. The Integrations experimental warning sits under Third-party integrations.

* fix(linear): use stable OAuth callback broker

* fix(chat): preview Linear issue attachments

The context switch missed linear-issue, so tsc treated the preview helpers as incomplete.

* fix(ui): restore Linear i18n parity and the #2903 sync harness

Turkish was missing the Linear dictionaries, and the subagent test still wrapped only SyncContext after reads moved to SyncRuntimeContext.

* fix(linear): drop changelog hunks and close review races

Keep changelogs out of this PR, restore CodeMirror ranges, ignore stale Linear list pages, and leave a persisted Linear tab open until auth has actually resolved.

* fix(linear): tint active issue filters and clear them in one click

* fix(markdown): read escaped brackets as text, not display math

`\[...\]` is display math in LaTeX and an escaped bracket pair in
CommonMark. The block tokenizer claimed every `\[`, so prose like
`[title \[Bug\] more](url)` was handed to KaTeX: "Bug" rendered as a
centered formula and the block token split the paragraph, tearing the
link into three pieces. Linear, GitHub and any other source that escapes
brackets the way CommonMark requires hit this.

Display math now has to own its line — `\[` starts one and `\]` ends
one. A formula on its own line still renders; `\[` mid-sentence stays an
escape, which is what CommonMark says it is and what prose almost always
means. Inline `\(...\)` keeps the same ambiguity, but inline math is
legitimately mid-sentence, so there is no position to judge it by.

Covered by regression tests, including the verbatim comment body that
surfaced this.

* feat(linear): make session status comments opt-in and public-only

A status comment lands in a Linear workspace the whole team reads, and
the link it carried pointed at whatever origin started the session —
usually loopback or a LAN address. Everyone but its author got a dead
link, and nobody had agreed to the comments in the first place.

Comments are now off until the user turns them on in Settings ->
Integrations -> Linear, and the check lives on the server: the event hub
posts completed and failure without going through the interface, so a
client-side gate would not hold. When the resolved origin is not
publicly reachable the server posts nothing at all rather than a link
only its author can open; `isPublicSessionOrigin` rejects loopback,
private LAN, carrier-grade NAT, link-local and single-label hosts. The
desktop deep-link origin is gone with it, since no one else can follow
one either.

The comment body also dropped the session title. It repeated the issue
the comment already sits on, and issue titles routinely carry brackets
("[Bug] ...") that broke the markdown link. The body is now one short
link, and `sessionTitle` is gone from the route, client and types.

Also caps the dedupe file at the newest 500 sessions; it grew forever.

* fix(linear): match the pull request panel and clear review findings

Comments in the Linear panel now render as the same avatar timeline the
pull request panel uses, with the shared time-format preference instead
of a raw locale string. Comment authors carry `avatarUrl`, which the
GraphQL selection was not requesting.

Review findings from the same pass:

- `status-runtime.js` hand-rolled `typeof` narrowing and failed the
  vendored anti-slop lint; it now parses through `parse.js` like every
  other file in the module.
- `useLinearAuthStore` turned any failed request into `connected: false`
  with `hasChecked: true`. Since the rail icon, the composer entry and
  the worktree option all gate on `connected === true`, one network blip
  hid Linear for the rest of the session, and Settings only re-checked
  when it had never checked. It now keeps the last known status and
  leaves `hasChecked` false so the next caller retries.
- `LinearIssuesView` (1096 lines) was a static import in `ContextPanel`,
  shipping in the main bundle although its rail icon stays hidden until
  a workspace is connected. It is lazy now, like `GitView`.
- Dropped dead code: the unused port helpers left over from the loopback
  callback, two re-exported default values nothing read, and a redundant
  export in `linkedIssues`.
- Integrations is no longer badged beta.

Bohdan Triapitsyn 48bcac1758 fix: harden and de-slop the merged sidebar/chat/settings batch
Post-merge follow-ups for #2740 #2735 #2734 #2690 #2676 #2738 #2684
#2689 #2733 #2739 #2462 #2687 #2736 #2618 #2697, plus three regressions
found while reviewing them:

- ctrl/cmd+digit while typing no longer switches session tabs (#2503 was
  still open in practice: the guard only covered the mod+alt surface binding)
- Shiki template-call sanitizer now covers every bundled grammar, including
  the js/ts aliases and embedding grammars; timed-out highlight requests are
  memoized and no longer cancel unrelated in-flight requests
- settings flush on suspend uses keepalive and also fires on Capacitor
  appStateChange; keeps the selected model persisted across mode switches
- remote-only branches fetch before checkout; range helpers fail clearly
- git status invalidation now fires for runtime adapters too
- settings number inputs and select triggers size in ch so they scale with
  the interface font
- recent-activity timestamps tick from one list-level ticker
- Markdown preview find goes through the shared find_in_file keybind with
  containment, no longer counts its own bar, and debounces observer runs
- #2676 reverted; #2524 fixed by fading the sticky header's own background
  instead of overlaying the content below it
- sticky group headers in the model picker and sidebar render again
  (oc-sticky-fade-scroller class restored after 9b9d7069c)
- project switcher names are left-aligned again (wrapper lost in 26dbc2f30)
- tool card quick-open icon is always visible and opens the same line as the
  expanded card's button
- tautological tests replaced or removed; new oxlint findings fixed

Bohdan Triapitsyn c788cd2237 fix(providers): keep declared env vars as a credential signal
Providers that read credentials from several environment variables
(Bedrock, Azure, Vertex) never get a single resolved Provider.key from
OpenCode, so dropping the env signal made them show "Credentials
missing", auto-open the auth panel, and hide their models even when
fully configured.

providerHasCredentials takes an envDeclared input again, and both
call sites in ProvidersPage pass whether the provider declares any
non-empty env var name.

Bohdan Triapitsyn 5332d124a6 fix(settings): let fixed-width controls scale with font size and density
Merge main and resolve conflicts in RemoteInstancesPage.tsx (re-applied
the width bumps by hand at their current locations) and
KeyboardShortcutsSettings.tsx (dropped the PR's hunk entirely: main
replaced that fixed-width input with ShortcutRecordingDialog, which has
no equivalent clipping at 200% interface font size).

herjarsa 7d20f2897a docs(providers): drop stale disconnect comment; rewrite changelog bullet
Two nits from the openchamber-bot review at 65668f14:

- The disconnect handler carried a stale comment that referenced a
  "removed:false payload gating" feature the rebase removed; the
  helper below it does not do that gating. Drop the comment.
- The Hide button onClick ran setAuthPanelDismissedForId inside the
  setShowAuthPanel updater (idempotent today, impure under StrictMode
  double-invoke). Move both calls outside the updater.

Also rewrite the [Unreleased] changelog bullet to describe the
user-visible change (Connected + models visible for options.apiKey
providers) instead of internal mechanics ("source refetch + optimistic
auth mark"), and broaden the wording from "after saving" to cover
OAuth, custom-provider, and disconnect paths.

herjarsa 65668f1453 fix(providers): wire optionsApiKey through both call sites; restore editable custom exemption
The previous fix added optionsApiKey to providerHasCredentials but
wired it through only one of the two call sites in ProvidersPage.tsx.
The auto-open effect at line 339 still omitted it, so a config-defined
provider whose only credential is options.apiKey would get the auth
panel force-opened on every selection while the summary beside it
said Connected; the dismissal resets on provider switch, so the
panel re-opens each time.

Restore the isEditableCustomProvider exemption that main's
requiresProviderAuth helper carried into both authStatusIncomplete
and shouldShowModelsSection. A keyless local custom provider (LM
Studio / Ollama style) regressed from 'models visible, no banner' to
'Credentials missing' with the models section hidden.

Rewire requiresProviderAuth (its only production consumer was lost
in the rebase) by delegating authStatusIncomplete to it. The helper
already encodes sourcesLoaded && !hasCredentials && !isEditableCustom
Provider, so the call site is one line and the contract matches main.

shouldShowModelsSection now accepts an optional isEditableCustom
Provider flag that lifts the credential gate for editable providers,
matching the exemption the ProvidersPage.test.ts:22 fixture asserts.

Tests: 15/15 pass. Was 14 in the previous commit; added one test
for the editable custom exemption.

herjarsa 75d93f79c0 fix(providers): treat options.apiKey as a credential in credential signal
The previous review flagged that providerHasCredentials misclassifies
working config-defined providers: Provider.key is only set by upstream
when exactly one declared env var resolves or an api-type auth.json
entry exists. Config providers only get options, so provider.<id>.
options.apiKey never reaches key. Result: a provider whose key is
embedded in opencode.json showed 'Credentials missing', lost the
Models section, and forced the auth panel open.

Add optionsApiKey to ProviderCredentialInput and check it in
providerHasCredentials alongside key and authSourceExists. This
matches what main's requiresProviderAuth helper used to do and
honors the OpenChamber docs contract that options.apiKey counts as
a usable login (walkthrough/DOCUMENTATION.md:134).

Wire the new field through ProvidersPage.tsx using a typed
indirection: the SDK Provider type does not yet expose options
publicly, so the read site casts the object to the known shape.
This keeps the call site type-safe without waiting for an SDK
update.

herjarsa addcbfe447 fix(providers): address bot blockers for stale Credentials missing state
- shouldAutoOpenAuthPanel: re-introduce isEditableCustomProvider exemption
  (was dropped when requiresProviderAuth was replaced). Custom providers are
  editable directly in the form and must not be force-opened into the auth
  panel on a stale sources snapshot.
- handleSaveCustomProvider + handleDisconnectProvider: route through
  applyConfigReloadOrRecordDeferred so an externally managed OpenCode that
  throws requiresManualRestart records deferred-restart guidance instead of
  toasting a misleading 'mutation failed' for a write that already persisted.
- handleOAuthConnected: call markAuthWriteSucceeded so the page does not stick
  on a stale 'Credentials missing' summary while the providers refresh lands
  (OAuth previously only updated the deferred-restart payload).
- Sources effect deps: add settingsDirectory back so a directory switch while
  the provider id is unchanged refetches the source snapshot (was swapped for
  providerSourcesRevision in the prior rebase).
- Cleanup: drop dead oauthCodes state and copyTextToClipboard import; restore
  the requiresOpenCodeRestartAfterOAuth import alignment.

13/13 ProvidersPage.test.ts still green.

Cursor AgentandSerhii Dziupin 82bb814af7 fix(providers): refresh credentials after auth save
Stop treating Provider.env length as credentials, refetch provider
sources after OAuth/API key writes, and respect an explicit auth-panel
Hide so OAuth-only providers do not stick on a stale "Credentials
missing" / empty-models state after a successful login.

Bohdan Triapitsyn b8465ae133 fix: harden and de-slop the merged contribution batch
Follow-ups promised on merge, plus review findings on the batch itself:

- chat: task-tool output now respects the 512KiB render cap; quick-open
  icon is visible at rest on coarse pointers and reachable by keyboard
  (row keydown no longer swallows inner-button Enter/Space); composer
  inline-code decoration drops the metric-shifting padding; a btw fork
  send carries only the boundary instruction, never the promotion notice
- sync: cascade revert/unrevert aborts busy descendants, busy state is
  read from every child store at the moment of use; rule 9 documents
  redo clearing all descendant revert markers
- electron: renderer recovery keeps memory-eviction (a valid
  render-process-gone reason) and both windows share one
  attachRendererRecovery helper
- vscode: process registry is a thin re-export of the web module
  (provider-env-aliases precedent) with ordered register/unregister
  writes and an awaited close
- server/cli: managed-process registry takes injectable deps (fixes the
  unreaped-orphans ReferenceError), corrupt settings errors name the
  file, getWorktrees test restores console.warn
- tests: module-mock harnesses removed (AgentsSidebar, SettingsView
  mobile focus — behaviors stay live but uncovered, accepted trade),
  QuestionMarkdown asserts rendered DOM
- i18n: German gains the debug-panel request keys, Japanese/German drop
  removed worktree keys, Ukrainian unit spacing fixed
- changelog: Copilot AI Credits entries (main + VS Code)

Bohdan Triapitsyn 7977842e1e feat(ui): redesign the default shortcut layout around a mod+k leader
Single chords stay for everyday actions; open/go actions move to two-step
mod+k sequences; held mod+digit switches header session tabs and held
mod+alt+digit switches context panel surfaces. Rare actions leave the
shortcut schema for the command palette, every remaining action ships with
a default binding, and stored overrides from the old layout reset once.
Key matching now follows the physical key on non-Latin layouts and for
Option-modified digits on macOS, including in the recording dialog.

Bohdan Triapitsyn f1c3870909 feat(ui): land the centralized shortcuts core from #2532 with review fixes
The schema/config/bindings/registry/dispatcher module, useKeybind hooks,
recording dialog, reworked shortcuts settings page, help dialog, and the
localized action labels — re-based onto current main rather than merged
(the branch predates 440+ commits including the session-tabs shortcuts).

Review fixes applied on top of the original:
- close_session_tab (alt+w) joins the schema with labels in every locale;
  it shipped on main after the PR's base and would otherwise silently die.
- switch_context_surface's special-case in conflict resolution is now a
  declared prefixStyle config property instead of a magic id string.
- Duplicate handler registration warns in dev builds.
- The risky-browser-shortcut warning inspects every chord and covers
  mod+q/d/h/j/o/u plus mod+shift+w/q.
- The dispatcher remembers which target armed a two-chord prefix so the
  window-level completion handler can distinguish a deliberate sequence
  from typing in an editable field (guard lands with the dispatch hook).
- Schema tests: unique normalized default bindings enforced, and the
  flat-file-era override format proven to keep resolving.

Bohdan Triapitsyn dac31ee026 feat(settings): add streaming auto-follow toggle
New Streaming section on the Chat settings page with a checkbox that
controls whether the viewport follows new content while a response
streams. Default stays on. With it off, the anchored user message still
parks at the top on send, but no glide or end-follow correction runs and
the list's maintain-scroll-at-end stays disabled; the scroll-to-bottom
pill and session open keep scrolling explicitly.

Persisted through desktop settings like the other chat toggles (auto-save
diff, authoritative apply, sanitize), registered in settings search, and
localized in every locale.

Bohdan Triapitsyn b6e14cbf21 refactor(settings): always-docked editor toolbar; reorder Navigation
The 'Always show editor toolbar' preference is gone — the docked
toolbar under the file tabs is now the only mode, and the floating
hover toolbar branch in the files editor (with its open-state and
outside-click machinery) is deleted. The stored preference is dropped
by a store migration and removed from desktop settings persistence,
settings search and every locale.

Navigation section order now reads: file editor keymap, auto-save,
terminal shell + login shell, Terminal Quick Keys, and the Session
tabs group last.

Bohdan Triapitsyn ce90e7e24f feat(settings): Session tabs toggle in General → Navigation
A new Session tabs group (web/desktop only) turns the header session
tabs off; disabled, the header renders the exact pre-tabs view — plain
session title with meta row and the always-visible session menu (the
same block VS Code uses). The Alt+W close-tab shortcut no-ops while
tabs are off. Registered in settings search with a matching anchor;
labels translated in all locales.

Bohdan Triapitsyn 841eca5720 feat(surface): switch app shells when the viewport crosses the phone threshold
The mobile-vs-desktop surface is stamped once at boot, so a browser
window narrowed past the phone threshold kept the desktop shell (and
its legacy squeezed layout) until a manual reload. A viewport watcher
now reloads into the other shell once the resize settles — the same
mechanism the old Settings toggle used. Fixed shells (Capacitor,
desktop, VS Code) and ?surface= overrides never switch.

With the new mobile app reachable this way, the old/new mobile layout
preference is gone: phones always get the mobile app.

Bohdan Triapitsyn b8716fe808 refactor(search): unify dropdown filtering on the shared ranked matcher
Branch, project, agent, model, provider, stash, SSH-host, skill-catalog
and archive filters each had their own toLowerCase().includes (or no
ordering at all); the git branch and gitmoji pickers also let cmdk
re-filter and reorder on top of the manual filter, silently dropping
rows. All of them now go through rankByQuery/matchesRankQuery: results
are relevance-ordered, multi-word queries match in any order, matching
ignores punctuation, and cmdk filtering is disabled where the ranked
list is already final. rankBranchesForQuery keeps relevance order
instead of re-sorting matches alphabetically; the model picker now also
matches model ids.

DOMPurify's default URI policy stripped href from anchors with custom
application schemes (obsidian://, vscode://, ...), so every app link
rendered in chat was dead across web, desktop, VS Code, and mobile.

- Classify safe app-link schemes in lib/url.ts (browser-handled,
  scriptable, webview-internal, network, and self-deep-link schemes
  stay excluded) and let openExternalUrl accept them
- Keep app-link hrefs through the markdown sanitize hook
- Intercept app-link clicks in the markdown renderer and route them
  through a confirmation dialog (Trust and open / Open once, dismiss
  to cancel) mounted in the desktop/web app root and the mobile shell
- Persist per-device trusted schemes in a zustand store; trusted
  schemes open without asking again

Add an App links section to Settings > General listing the application
schemes trusted on this device with a delete action; removing a scheme
restores the confirmation dialog for it. Register the section in
settings search.

Bohdan Triapitsyn 2740c7b3a2 feat(projects): pin a thinking level next to a project's model
A project could pin the model new chats start on, but not the level to
run it at: the default cascade dropped any variant as soon as a project
model won, and only ever considered the global one — which belongs to
the global model.

Projects now carry `defaultVariant` alongside `defaultModel`, stored and
sanitized only next to that model, and the cascade passes it through.
Both controls sit in one "Defaults for new chats" group laid out like the
Sessions defaults, and the level appears only for models that offer them.

Bohdan Triapitsyn e2e55756e2 fix(settings): stop defaults from repointing a chat that has its own model
Changing the default model, variant or agent wrote the chat's live
selection unconditionally. For a session carrying its own choice the
effect was mostly transient — ModelControls restores that choice on the
next hydration — but a message sent right after the change went out on
the new model.

The guard is the same pair of signals ModelControls restores from
(shouldPreserveManualModelOverride): a manual selection plus a saved
selection for the current session. Chats following the default still
switch immediately.

Bohdan Triapitsyn 0079347edc refactor(settings): scope the settings project selector to settings
Picking a project in Settings called setActiveProject, which relocates
the chat, the session list, the file tree and the Git surface. Reading
another project's MCP servers or agents moved the user's whole app.

It had to, because the configuration stores resolved the directory
themselves from the active project and held one flat list. Each of them
now takes an explicit directory — omitted still means the active project,
so every caller outside Settings is unchanged — and keys loaded data by
directory next to a flat mirror of the active project. Chat, autocompletes
and pickers keep reading that mirror; a load for another directory writes
only the map. A failed load restores that directory's previous list.

Settings resolves its own directory through useSettingsDirectory, backed
by a session-local settingsProjectPath that follows the active project
until the user picks something else.

Bohdan Triapitsyn f7a0d1f0f6 feat(ssh): rework remote instance setup and fix remote installs
Adding an SSH connection now starts from the hosts in the SSH config
instead of a blank command field, ports and install options sit behind
Advanced settings, and each connection reports one of three states with
the failure text and an action that resolves it.

Remote installs no longer touch the root-owned global npm prefix: npm is
pinned to a prefix under $HOME and bun is resolved at its known location,
because an SSH login shell exposes neither on PATH. The opencode CLI is
resolved the same way and handed to the remote server through
OPENCODE_BINARY, and the server is started and stopped through the
resolved binary rather than PATH — the HTTP shutdown route sits behind UI
authentication and never stopped anything.

A managed remote server can also be published to the remote machine's own
network. That requires a UI password, enforced in the form and again in
the SSH manager.

Bohdan Triapitsyn 1ed3f1f575 feat(skills): curated GitHub catalog redesign (#3016)
* feat(skills): remove ClawHub catalog integration

Drop the ClawHub registry as a skills catalog source across web server,
shared UI, VS Code, docs, and locales. The catalog now serves git-based
sources only: the curated Anthropic repo and user-defined repositories.
Also removes the now-unused adm-zip dependency.

* feat(skills): redesign catalog around curated GitHub repositories

Replace the single-source dropdown with a card grid of curated GitHub
repositories (Anthropic, OpenAI, Cursor pstack/skills, Matt Pocock) plus
user-defined sources. Source cards show skill counts, GitHub stars, and
last-updated time; a global search covers all loaded sources.

Server: curated sources gain GitHub repo metadata (stars, pushed_at)
fetched best-effort with a 3-hour in-memory and on-disk cache; scans
run through a concurrency-limited, deduplicated cache with 3-hour TTL
persisted across restarts. Refresh still bypasses the cache.

Shared UI: source cards, global search with clear button, per-skill
GitHub links, install/installed states. VS Code curated list updated
to match. All new copy translated across 12 locales.

* fix(skills): address catalog review findings

- GitHub metadata fetch timeout drops to 1.5s (under the catalog
  client's 3s deadline) and failed lookups cache briefly (5 min) so
  repeated catalog loads do not re-hit a failing API.
- Disk cache files are written with owner-only permissions (0o600);
  rename preserves the mode.
- loadSource deduplicates concurrent in-flight requests per source and
  the shared isLoadingSource flag now clears only when the last active
  source load finishes.

Bohdan Triapitsyn 6a09c63392 feat(small-model): resolve plugin-registered providers from the running OpenCode
Plugin providers are registered from a plugin's `config` hook and credentialed
from its `auth` loader, both inside the running OpenCode process. Nothing about
them reaches `opencode.json` or `auth.json`, so resolution that only reads files
could not see them: selecting such a model failed with "has no known API base
URL" while the same model worked in chat (#2666).

`GET /provider` is where that state is visible. A new `runtime-providers`
module keeps one cached snapshot of it and reports, per provider, the
credential and endpoint OpenCode itself resolved. Credential resolution becomes
config -> runtime -> auth.json, and endpoint resolution config -> openai default
-> runtime -> models.dev catalog.

Providers with a dedicated wire format (Copilot, ChatGPT-plan OpenAI, Anthropic,
Google) are excluded from the runtime credential: for them OpenCode reports an
OAuth access token that their real transport does not accept.

opencode zen is excluded when the user has no zen login. OpenCode then reports
the sentinel `apiKey: "public"` and trims its catalog to free models that run on
its own infrastructure; the sentinel is never read as a credential.

Claude Code stays refused for background actions even when a plugin publishes an
OpenAI-compatible endpoint for it, because that endpoint is a facade over the
Claude Agent SDK and spawns the CLI per request.

No capability probe. Asking `GET /models` does identify a plugin whose protocol
lives in its own `fetch`, but measured across the 166 providers with an `api`
URL in the models.dev catalog it also denies six that work and simply have no
`/models` route. A provider that vanishes from the picker explains nothing,
while one that fails on use says why, so availability stops at credential and
endpoint.

The same list drives the Small Model and Changes Walkthrough pickers.

Validated against a real OpenCode with four plugin providers loaded: offered
providers went from 3 to 7, zen and Claude Code stayed out, and a generation
through a plugin-backed model that previously failed now returns.
