Use current working directory for issue lookup before active project fallback

Extract normalizeWindowsDriveLetter to shared pathUtils module
Normalize workspaceFolder to uppercase drive letter to match OpenCode
server's path normalization. This fixes session visibility issues
where VS Code extension sessions don't appear in CLI/desktop and vice
versa due to case-sensitive directory comparison on Windows.
Changes:
- Create pathUtils.ts with shared normalizeWindowsDriveLetter function
- Update opencode.ts to import from pathUtils instead of inline definition
- Update ChatViewProvider.ts, AgentManagerPanelProvider.ts,
SessionEditorPanelProvider.ts to import from pathUtils
* fix: preserve lastEventId in SSE path and add proxy heartbeat
- Extract event.id from SSE stream events in event-pipeline.ts so that
reconnects carry the correct Last-Event-ID header for gapless replay.
- Emit :heartbeat comment every 20s in the direct SSE proxy to keep
the UI heartbeat watchdog from aborting idle connections.

Pinned folders in the Add Project dialog now show an expand/collapse
arrow. When expanded, subdirectories are lazily loaded and rendered as
a recursive tree, allowing users to browse into pinned parent folders
(e.g. a 'local-dev' folder) and pick any nested subfolder as a project.

Add inline thinking-mode chips that show each model's remembered or default variant, apply model and variant together on tap, and fall back to a dedicated overflow panel for larger variant sets. Also keep favorites and recents searchable on mobile and fix clearing remembered default variants so the picker stays consistent across sessions.

areRenderRelevantPartsEqual did not compare the tool part output field,
so when the AI updated a todo list (e.g., marking items completed) the
ToolPart memo comparator returned true and skipped re-render. The UI
stuck showing stale task status.
Fix: add output reference comparison for tool parts in
areRenderRelevantPartsEqual so streaming tool output updates trigger
re-renders correctly.
